Waivers

INDIAN NATIONS YOUTH SPORTS - TERMS & CONDITIONS:

By checking the box below, I the registrant and/or legal guardian of the registrant am legally agreeing to all terms and conditions set forth by Indian Nations Youth Sports and the individual club and/or organization in which I am registering.

I am agreeing that I am at least 18 years of age and the legal guardian of the person/persons registering hereof. I further agree that any information provided in the following pages is truthful and accurate to the best of my knowledge.

I also state that any falsification of documents, illicit or immoral activities will be punished pursuant to the by-laws of the proper governing authorities, and that such activity will lead to penalties including: no reimbursement of funds and partial or permanent expulsion from the affiliation/organization I am applying for.

Right to Refuse Service

By accepting the terms of membership in the Indian Nations Youth Sports (INYS) programs I acknowledge that the INYS is a private organization and that it reserves the right to refuse membership and/or participation to any person whose actions hinder the activities of the INYS and that such determination will be at the sole discretion of Indian Nations Youth Sports.

Arbitration

IT IS AGREED THAT ANY MATTER IN DISPUTE BETWEEN YOU AND INDIAN NATIONS YOUTH SPORTS SHALL BE SUBJECT TO ARBITRATION AS AN ALTERNATIVE TO COURT ACTION PURSUANT TO THE RULES OF THE AMERICAN ARBITRATION ASSOCIATION, A COPY OF WHICH IS AVAILABLE ON REQUEST FROM THE COMPANY. ANY DECISION REACHED BY ARBITRATION SHALL BE BINDING UPON BOTH YOU AND THE COMPANY. THE ARBITRATION AWARD MAY INCLUDE ATTORNEY'S FEES IF ALLOWED BY STATE LAW AND MAY BE ENTERED AS A JUDGEMENT IN ANY COURT OF PROPER JURISDICTION.

Attorney's fees

The Parties agree that if Indian Nations Youth Sports prevails in any litigation or arbitration brought arising from any dispute, then INYS shall be entitled to recover all costs, expenses and attorney’s fees associated therewith.

PARTICIPATION AUTHORIZATION AND RELEASE OF LIABILITY:

For himself/herself and for his/her spouse and for his/her child and other children hereby represents and agrees to the following:
My child has my permission to participate in all sponsored or endorsed activities. The Association and its Board Members and Directors, Officers, Coaches, Coaching Staffs, agents and licensees are hereby released from any and all liability or responsibility for any injury that may occur to my child, to me, to my spouse and any of my other children resulting directly or indirectly from my child's participation in activities including, but not necessarily limited to, league tournaments, practice games, practices, transportation to and from games and tournaments or otherwise and the use of practice facilities, games facilities, concession facilities or any other facility.

AUTHORIZATION FOR EMERGENCY TREATMENT

In the event of a medical emergency, I hereby authorize my child to be transported to the nearest emergency room or medical facility. I also authorize officials to secure the use of an ambulance for transporting my child to the hospital and/or to administer first aid treatment as necessary. I further authorize any physician, surgeon or dentist of the nearest emergency medical center to administer any emergency treatment procedure or medicine necessary or advisable, when accompanied by an adult. I further agree to pay the hospital, doctors and ambulance fees for all services rendered to the above named child. I request that this authorization remain in force from this date until the end of the calendar year unless notified in writing of a change by me.

YOUTH SANDITE CODE OF CONDUCT
“24 Hour Rule”
Parents sometimes disagree with a coach's decision, coaching style, officials' decisions or game rules. Parents have to understand that the coach represents the entire team, not only one player, and must make decisions from the team’s perspective first and foremost. For parents, it is important to separate their child's sports development from game emotions. For this reason, we have adopted the "24 HOUR RULE", which simply states that the coach/staff will not discuss game situations until 24 hours after the game has passed. This rule helps to move the discussion away from the presence of the players and allows all parties to have time to put things in perspective and "cool off’ if necessary.
Attendance Policy:
Practices are not suggested but mandatory. Failure to come to practice may result in reduced playing time and sitting out of entire games. Any player missing two practices during a given week may be required to sit out for the upcoming game. Any player missing all practices for a given week will not be able to play in the upcoming game.
Parent/Guardian & Spectator Policy:
Spectators are expected to follow their roles in the INFC Rule Book and overall be a role model for all the football players on the field. This policy also applies for any spectators (including parents/guardians/family friends). In the event any person in the stands/sidelines of a practice or game, that goes against the rules and zero tolerance policy, will be asked to leave. If the behavior continues, the individual(s) may be banned from attending games/practices. Parent behavior may result in player suspension. If a parent/guardian/spectator is removed from the game, both the parent associated with the ejected person and the player will not be allowed to attend the next game. This rule also applies to guardians and family friends mentioned above. Spectators will not argue/yell/question the officials before during or after play. Dealing with the officials is exclusively handled by the head coach of the team. Parents are asked to cheer in a positive manner for their children. Leave the coaching/correcting to the coaches and the officiating to the officials. This is a zero-tolerance policy. Keep it positive or keep it quiet. There is a process for feedback on officials that the head coach is aware of.
We have a zero-tolerance policy for stealing, bullying, unsportsmanlike conduct or behavior by an individual (players, participants, coaches, officials, spectators, or parents), or causing physical harm to others. SSYFA reserves the right to suspend players from any activity without refund, if these behaviors are identified while participating. Depending on the severity of the situation, a player may be given a partial suspension from the program, with an opportunity to improve their behavior. If the situation is a distraction to the team or a safety issue, the player may be subject to a permanent suspension from the football program. No refunds will be issued if any suspensions, or removal from the program takes place.

I have read the Rules and Regulations and Code of Ethics, as stated above by SSYFA. I hereby agree to abide by said rules and regulations. I have also read and fully understand the penalties prescribed for violation or non-compliance of said rules and regulations and by checking the box do agree to these terms and conditions.

SSYFA Refund Policy

Cancellation Refund Policy:
Cancellation requests must be sent via email only to coachouryouth@gmail.com. Refunds are based upon the number of days the request is received after the official start date (first practice) of the season. Refunds are sent via check to the primary parent and address listed first on your registration form once equipment is returned.

1-7 days - 100%
8-14 days - 50%
15-21 days - 25%
After 21 days - No refund

Equipment Rental Policy

I understand that the football equipment issued for the season is on loan from SSYFA and must be returned at one of the two equipment checkout dates to be scheduled after the conclusion of the season. Failure to return the equipment at these scheduled events will incur a $300 missing equipment fee and registration hold for the family until paid. I understand this is not negotiable if equipment is turned in late or lost.

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate on behalf of the Sand Springs Youth Football Association, a member organization of; the Indian Nations Football Conference, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Sand Springs Youth Football Association, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)
This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.
